This Opportunity:
The Community Specialist is primarily responsible for developing a positive relationship with the individual receiving services and their family in a work, home, school, or community setting while assisting them in achieving their personal dreams and goals as designated in the individual goal plans, as well as providing periodic relief to the caregiver.What You'll Do:
o Assess each person receiving services for their hopes, dreams, and desires and provide supports that facilitate achievement.o Ensure that the rights of each person receiving services are protected and promoted at all times. Assist people in developing advocacy skills, participating in advocacy efforts, and utilizing personal advocates, as well as, provide periodic relief to the caregiver.o Serve as a resource for individual receiving services on community agencies, services, and supports that can meet identified needs/goals.o Supervise, educate, and monitor (as needed) individual receiving services in work, home, school, or community type settings.o Provide one-on-one support as needed to meet the emotional, physical, and medical needs of each person supported.o Maintain a safe environment for community, employees, and individual receiving services by practicing safety procedures.o Facilitate person-centered, effective, positive relationships with individual receiving services using positive approaches that promote self-determination in all areas of life.o Provide input and recommendations into assessment and planning processes, and development of the individual's plan.o Implement person's plan fully including but not limited to respite plan, within the requirements of the service definition, and document as required by the service definition and agency policy. Accurately complete all required documentation, including but not limited to, goal completion, actions toward outcomes, incident reports, timesheets, etc.o Complete daily progress notes and communication log to assure appointments, goals, and interests are met and/or daily documentation.o Assist in maintaining all necessary records, daily attendance, check sheets, production sheets, etc.o Substitute in-house or in the community as demands occur.o Assist new staff and/or current staff with orientation, mentoring, and training.o Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.o Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.o Follow service definition guidelines for services being provided.o Perform all other duties as assigned by the supervisoro Driving and travel may be requiredEducation We're Looking For:

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
